The National Transportation Safety Board (NTSB) said investigators were on scene of a crash that took place near Ronald Reagan Washington National Airport (DCA) Wednesday night. WASHINGTON D.C., DC — Authorities believe there are no survivors after a mid-air collision involving an Army Black Hawk helicopter and an American Airlines regional jet over the Potomac River in Washington D.C. Wednesday night. There were 60 passengers and four crew members on the plane and three service members on the helicopter.

Secretary of Transportation Sean Duffy said the midair collision should not have happened, and vowed reforms to "make sure that these mistakes do not happen again and again." The cause of the crash is under investigation.
